Per the provisions in Idaho Code § 39-118(2)(d), the City of Meridian has retained the services of
Keller associates .as its Qualified Licensed Professional Engineer (QLPE), to verify that the plans and
specifications proposed for the above -referenced project comply with the IDEQ Facility and Design
Standards (IDAPA 58.01.16 and 58.01.08) and the applicable Meridian standards, and to approve the
project for construction. By virtue of their attached letter, they have reviewed and approved the above
referenced plans and specifications as complying with the Facility and Design Standards, and
Engineering Standards of Care as it relates to the reviewed elements.
The above referenced project complies with the City of Meridian's current Wastewater Facility Plan,
and Sewer Master Plan, and our system has adequate capacity to accept sewage wastes for treatment.
These determinations have been made by use of computer models developed for our Sewer Master
Plan. This project is also within the City's urban service planning boundary. Our Domestic Water
distribution system can provide water service and the minimum required fire flow (1000 GPM) and the
minimum required residual pressure of 20 PSI. These determinations have been made by use of
computer models developed for our Water Master Plan.
The City of Meridian will own and operate the wastewater collection and domestic water distribution
systems within this project upon final inspection and acknowledgement by formal acceptance letter. In
essence, the City of Meridian "Will Serve" the project.
Attached for your records is a complete and accurate copy of the plans and specifications reviewed in
connection herewith, stamped "Approved for Construction."

Accommodations Subdivision
Keller Associates has reviewed the Accommodations Subdivision construction plans dated June
10, 2015. The plans are approved for construction as allowed under Idaho Code 39-118(2)(d).
The completed "Qualified Licensed Professional Engineer (QLPE) Review and Approval
Checklist" (QLPE Checklist) is attached. The QLPE Checklist was prepared to meet the
transmittal letter requirements specified in IDAPA 58.01.08.504.03(b) and 58.01.16.400.03(b).
The plans were marked as "Approved for Construction".
By stamping and signing the Construction Drawings, the Registered Professional Engineer is
responsible to ensure that said construction drawings conform to all City standards, ordinances,
and policies (and State laws and statutes). Any variances or waivers to these standards,
ordinances, or policies must be specifically and previously approved in writing by the City.
Acceptance of the above referenced Construction Drawings does not relieve the Registered
Professional Engineer of those responsibilities.
Please note that the certifications required for Sections II(B) and (C) of the QLPE Checklist must
be satisfied with a will -serve letter and declining balance statement from the City.
Electronic Files have been uploaded to the City of Meridian's FTP site in accordance with the
terms of the Contract and we are returning one copy to the City for your records. Additionally
we are returning one set of the original redlines.

SANITARY SEWER EASEMENT
THIS INDENTURE, made this _ day of , 20 between Summit Equity, LLC./David
Alexander, the parties of the first part, and hereinafter called the GRANTORS, and the City of
Meridian, Ada County, Idaho, the party of the second part, and hereinafter called the GRANTEE;

WHEREAS, the GRANTORS desire to provide a sanitary sewer right-of-way across the
premises and property hereinafter particularly bounded and described; and
WHEREAS, the sanitary sewer is to be provided for through an underground pipeline to be
constructed by others; and
WHEREAS, it will be necessary to maintain, service and subsequently connect to said
pipeline from time to time by the GRANTEE;
NOW, THEREFORE, in consideration of the benefits to be received by the GRANTORS,
and other good and valuable consideration, the GRANTORS do hereby give, grant and
convey unto the GRANTEE the right-of-way for an easement over and across the following
described property:

The easement hereby granted is for the purpose of construction and operation of a sewer line
and their allied facilities, together with their maintenance, repair, replacement and
subsequent connection at the convenience of the GRANTEE, with the free right of access to
such facilities at any and all times.
TO HAVE AND TO HOLD, the said easement and right-of-way unto the said GRANTEE,
it's successors and assigns forever.
IT IS EXPRESSLY UNDERSTOOD AND AGREED, by and between the parties hereto,
that after making repairs, performing maintenance, replacements or subsequent connections
to the sanitary sewer mains, GRANTEE shall restore the area of the easement and adjacent
property to that existent prior to undertaking such procedures. However, GRANTEE shall
not be responsible for repairing, replacing or restoring anything placed within the area
described in this easement that was placed there in violation of this easement.

THE GRANTORS hereby covenant and agree that they will not place or allow to be placed
any permanent structures, trees, brush, or perennial shrubs or flowers within the area
described for this easement, which would interfere with the use of said easement, for the
purposes stated herein.
THE GRANTORS hereby covenant and agree with the GRANTEE that should any part of
the right-of-way and easement hereby granted shall become part of, or lie within the
boundaries of any public street, then, to such extent, such right-of-way and easement hereby
granted which lies within such boundary thereof or which is a part thereof, shall cease and
become null and void and of no further effect and shall be completely relinquished.
THE GRANTORS do hereby covenant with the GRANTEE that they are lawfully seized and
possessed of the aforementioned and described tract of land, and that they have a good and
lawful right to convey said easement, and that they will warrant and forever defend the title
and quiet possession thereof against the lawful claims of all persons whomsoever.

About this form
• This checklist is optional to use and is guidance from DEQ to qualified licensed professional
engineers (QLPE). This checklist describes the required elements of a QLPE project transmittal letter
and is intended to assist DEQ in the tracking of drinking water and wastewater projects, as well as
assist the QLPE in reviewing and approving certain types of Plans and Specifications.
• This checklist should be filled out by the qualified licensed professional engineer (QLPE) when the
QLPE is reviewing and approving a project for construction under Idaho Code § 39-118(2)(d). If the
project requires DEQ approval prior to construction, the QLPE shall direct the Design Engineer to
submit the plans and specifications through the normal DEQ review process.
• This checklist may be used in place of a transmittal letter from the QLPE.

IV. SANITARY RESTRICTIONS
If the project includes drinking water and wastewater main extensions that will be reviewed and
approved separately, the two Approving Agencies must coordinate the review and approval as it
pertains to the lifting of sanitary restrictions.
Recommendation Yes

V. NOTE ON RULES AND STANDARDS
This checklist addresses the majority of common items from the Idaho Rules for
Public Drinking Water Systems (IDAPA 58.01.08), the Wastewater Rules (IDAPA
58.01.16, for QLPE approval of projects. However, this checklist is not all-inclusive,
and users are expected to fully understand the rules and standards, apply them
where necessary, and request interpretations from DEQ if there are any questions.
DEQ regional offices may have additional written information that will assist in the
design/approval process.
