Under the
MASTER AGREEMENT FOR PROFESSIONAL SERVICES
BETWEEN
CITY OF MERIDIAN (OWNER) AND CIVIL SURVEY CONSULTANTS, INC (ENGINEER)
This Task Order is made this 13-1" day of October, 2014 and entered into by and between the City
of Meridian, a municipal corporation organized under the laws of the State of Idaho, hereinafter
referred to as "City", and accepted by CIVIL SURVEY CONSULTANTS, INC, hereinafter referred
to as "Engineer" pursuant to the mutual promises, covenant and conditions contained in the Master
Agreement between the above mentioned parties dated October 25, 2012. The Project Name for
this Task Order 10529 is as follows:

PROJECT UNDERSTANDING
To provide Qualified Licensed Professional Engineer (QLPE) services on an as needed basis for
the City of Meridian Public Works Department.
SCOPE OF SERVICES
Consultant shall provide services per the following:
• $125 base fee per plan set and $75 per reviewable sheets. Base fee covers plan
pickup from the City, delivery of reviewed plans to the City and preparation of
review letter. Reviewable sheets do not include sheets such as the cover or title
sheet, note sheets, standard detail sheets, topography sheets and so on.
• Projects may only contain water and/or sewer main extensions in accordance with
Idaho Department of Environmental Quality (IDEQ) QLPE eligibility requirements.
• Plans shall only be reviewed for compliance with IDEQ separation rules and any
other required items per IDEQ QLPE review and approval checklist. Plans shall
not be reviewed for compliance with City of Meridian standards and
specifications.
• Plans shall be reviewed and returned to the City within 5 business days starting
on the day after plans are picked up from the City by our office.
• Once contacted by the City we shall pick plans up from the City the same or
following business day.

Fees will be billed for actual plan sets and reviewable sheets at the rates listed below. Fees will be
billed monthly for completed QLPE reviews only. Services will be completed on an on call basis.
