Description: 1) Provide 5 sets of approved pians and spedUcationsfor the construction process. 2) Provide project Informational Flyer for affected residents. 3) Answor RFI's from tho conslraclor during the bidding and construction process. 4) Provide one set of construction stakes. 5) Review material submittals from the contractor. 6) Construction assistance to provide clarillcallon on pians and specs 7) Attend Pre construction conference. 8) Prepare record drawings to meet City of Meridian's Acceptance of RecordlEleclronic Drawing Standards. Reason for Change Order: Original task order did not include construction services. The design and construction ware originally projected to be one year apart from design to construction. The timeline has moved up so construction is In the fiscal year directly following design. Laughridge Ave to End, Waterline Replacement project by including bidding and construction support services to the existing contract. The original task order was executed March 4, 2014 and is subject to the mutual promises, covenant and conditions contained in the Master Agreement dated May 7, 2010. SCOPE OF WORK - Services to be performed by Keller Associates: Keller Associates will furnish services specifically limited to the following. 1. PROPERTY OWNER COORDINATION Services to be Performed by Keller Associates: Keller Associates will fumish the following: A. Property Owner Communication - Construction Phase After the City awards the construction contract to the successful bidder, Keller Associates will, In coordination with the Contractor and City staff, prepare a project Informational flyer for distribution to affected and potentially affected property owners, residents and businesses. For properties Immediately fronting the water main replacement areas, the flyers will mailed by the City of Meridian. The flyer will also be mailed to other properties within 300 feet of the project areas. The informationalfiyer will provide: • Reason for project Projected dates for work in each area • Description of work, material staging area(s), work set-up areas, etc. • Process by which Contractor will notify property owners of Interruption of water service • Expected duration and time of day of water service Interruption and City or Contractor mitigation efforts, if any (e.g. temporary sanitary facilities, temporary water connections) • General trafflc control plans and restrictions • Contact Information for Contractor and key City staff Deliverables: • Construction Phase project information sheet/maller Assumptions: • Keller Associates to provide updated text and coordinate edits, revisions with City • Keller Associates to provide design/layout; Keller Associates will produce Flyers 2of 3 • PDF versions of information materials provided to City staff for posting on City web site An estimated total of up to 40 information flyers may be mailed 2. CONSTRUCTION SERVICES A. Construction Documents Consultant will update the special provisions to reflect the property owner communication and construction staking services to be provided by Keller Associates. Keller will provide 5 hard copies and one electronic copy (PDF format) of the Approved Plans, Special Provisions, and technical Specifications for the City's incorporation into its construction documents. B. Preconstruction Conference Keller Associates will attend a pre -construction conference for the project with the Contractor, City, ACRD, and other parties. The City will prepare the agenda, coordinate with the various parties, and conduct the meeting. C. Construction Staking Keller Associates will place offset stakes at event points, valves, approximate service tees and hydrants for the length of the project. The vertical depths will be the responsibility of the contractor. Stakes will be placed one time. Any restaking will be the responsibility of the contractor at contractor's expense. D. Construction Assistance Keller Associates will be available during construction on an as -needed basis as requested by the City to answer questions or provide clarifications of the Plans, Special Provisions or Technical Specifications. Approximately 25 hours of construction administrative support was included.. No on-site construction observation services are anticipated. E. Submittal Review Keller Associates will provide technical review of submittals for project material as submitted by the contractor. F. Record Drawings Keller Associates will prepare record drawings from drawings and data received from the Contractor and City's staff per the City of Meridlan's Acceptance of record I Electronic Drawing Standards. Keller Associates will rely upon the information provided by the Contractor and the City in preparing the record drawings. 3 of 3
