Pursuant to the
MASTER AGREEMENT FOR PROFESSIONAL SERVICES
BETWEEN
CITY OF MERIDIAN (OWNER) AND DC ENGINEERING (ENGINEER)
This Task Order is made this 5th day of December, 2014 and entered into by and between the
City of Meridian, a municipal corporation organized under the laws of the State of Idaho,
hereinafter referred to as "City", and accepted by DC Engineering, hereinafter referred to as
"Engineer" pursuant to the mutual promises, covenant and conditions contained in the Master
Agreement (Category 7a) between the above mentioned parties dated September 18, 2012.
The Project Name for this Task Order 10382.d is as follows:

PROJECT UNDERSTANDING
The Engineer shall provide professional engineering services related to the construction of
fiber optic conduit and cable from Meridian City Hall to the Water Department and the
Wastewater Treatment Plant.
SCOPE OF WORK
Task 1 —Administration
1.1 Submit written communication through the City's project management software.
This includes but is not limited to the following:
• Reports
• Drawings
• Requests for Information
• Request for Action by City
• General Project Documentation & Communication
• Pay Applications

Task 2 — Construction Services
2.1 Provide up to six copies of final construction plans for distribution during
bidding and construction phases.
2.2 Provide one set of construction stakes for the design improvements. We
anticipate that all staking will be completed in two trips to the site. Horizontal
control shall be provided for each new junction box.
2.3 Engineer will attend the pre -construction meeting.
2.4 Answer RFPs from contractor during bidding and construction about the design.
2.5 Review material specifications submitted by contractor.
2.6 Prepare record drawings meeting the City of Meridian Acceptance of
Record/Electronic Drawings, dated October 25, 2012.
ASSUMPTIONS
Staking will be completed within 48 hours when requested in writing by the
Contractor.
The City shall provide one set of neatly redlined plans depicting any changes to
the plans to be incorporated into the record drawings.
This scope does not include any survey monument replacement if monuments
are disturbed.
TIME OF COMPLETION and COMPENSATION SCHEDULE
The need for services under this agreement are dependent on the project being put out to bid
and constructed by the City. Services shall be provided as necessary to assist the City with
the bidding and construction processes.
The Not -To -Exceed amount to complete all services listed above for this Task Order No.
10382.d is Nine Thousand Four Hundred Thirty -Eight dollars and 00/100 ($9,438.00). No
compensation will be paid over the Not -to -Exceed amount without prior written approval by the
City in the form of a Change Order. The hourly rates for services and direct expenses shall be

compensation will be paid over the Not -to -Exceed amount without prior written approval by the
City in the form of a Change Order. The hourly rates for services and direct expenses shall be

billed per the approved Rate Schedule and Master Agreement (by this reference made a part
hereof) and will be the basis for any additions and/or deletions in services rendered. Travel
and meals are excluded from this Task Order unless explicitly listed in the Scope of Services
AND Payment Schedule.
* Printing costs will only be reimbursed at direct cost and only with receipt.
