HomeMy WebLinkAboutExecuted Task Order 10507.e Strata WWTP Safety Improvements Special InspectionsTASK ORDER NO. 10507.e Under the MASTER AGREEMENT FOR PROFESSIONAL SERVICES BETWEEN CITY OF MERIDIAN (CITY) AND STRATA (ENGINEER) This Task Order is made this 3 0 Oday of December, 2014 and entered into by and between the City of Meridian, a municipal corporation organized under the laws of the State of Idaho, hereinafter referred to as "City", and accepted by STRATA, hereinafter referred to as "Engineer" pursuant to the mutual promises, covenant and conditions contained in the Master Agreement (Category 4a) between the above mentioned parties dated October 1, 2014. The Project Name for this Task Order 10507.e is as follows: CITY OF MERIDIAN MERIDIAN WWTP - SAFETY IMPROVEMENT PROJECT - SPECIAL INSPECTIONS PROJECT UNDERSTANDING We prepared our proposed scope of services and the corresponding fee estimate based on our review of the project plans prepared by Mountain Waterworks (civil), dated October 6, 2014; and Axiom (structural), dated October 8, 2014. We understand the proposed construction will be comprised of the fabrication and installation of multiple stair and platform assemblies. We also understand that the stair assemblies will be placed atop existing and new cast -in-place concrete and attached using post -installed fasteners. SCOPE OF SERVICES At the time of the preparation of this Proposal, STRATA did not have the opportunity to review the contractor's construction schedule. Therefore, we are only able to provide a preliminary fee estimate which may be subject to change due to construction schedule, workforce, weather, or jurisdictional requirements which cannot be anticipated. We have based our attached Preliminary Fee Estimate (Attachment A) on certain assumptions; specifically, the quantity of site visits for various construction aspects which we believe are consistent with current construction practices. These assumptions are detailed on the corresponding Estimate. We request the opportunity to review this document as it becomes available, and make appropriate modifications to our Estimate. On this project we anticipate STRATA's primary scope of services will consist of the following activities: 1) Observation and compaction testing during soil improvements, site grading, utility trench backfill and earthmoving activities. Task Order 10507.e Meridian WWTP Safely Improvement Project Special Inspections Page 1 of 6 STRATA 2) Special inspection of the placement of reinforcing steel and items embedded in concrete. 3) Sampling and quality control testing of concrete. 4) Special inspection during placement, consolidation, and curing of concrete. 5) Structural steel construction, including special inspection of field and in -shop welding. 6) Special inspection during the installation of post -installed -anchors (chemical epoxy and expansion type anchors). 7) Sampling and laboratory testing of various construction materials (i.e. concrete, soils, aggregates, etc.). SERVICE DESCRIPTION STRATA has assigned Mr. Don Hill as our Project Manager to provide supervision and technical oversight of our services. Mr. Hill will be responsible to interact with your designated project representative to coordinate and schedule our special inspection, field observation and laboratory testing services. Staff assignment will be dependent on the nature of the testing required. Our intention is to assign a primary special inspector with multiple certifications and experience that will provide the majority of the required testing; thereby increasing the continuity, effectiveness and efficiency of our services. This individual will be supported by other testing professionals as dictated by the project schedule and testing requirements. Pre -Construction Meetings STRATA's Project Manager will attend a pre -construction conference with you, appropriate subcontractors, and the local jurisdictional agency to review the inspection and materials testing requirements for the project and to establish report distribution protocol. If requested, we will also attend subsequent project meetings with the contractor and subcontractors for specific construction applications (i.e. earthwork, concrete, and structural steel) as you deem necessary. Earthwork Observation and Testing Services We will provide field professionals to perform observation and compaction testing services during site grading, placement and compaction of structural fill, backfilling of utility trenches, soil improvement activities, placement and compaction of subgrade and aggregate base material. Compaction testing will generally be performed as required by the project specifications. Our personnel will sample on-site and imported materials used for backfill and transport these samples to our laboratory for appropriate testing. These visits will be provided on a periodic basis which will be coordinated with the contractor's project superintendent. Special Inspection of the Placement of Reinforcing Steel and Embedded Items We will provide and International Code Council (ICC) certified special inspectors to verify the placement of reinforcing steel and embedded items prior to the placement of concrete. Our inspection of reinforcing steel and embedded items will include verification of type, grade, size, cleanliness, lap splice, clearance, coverage, general placement of reinforcing steel/embedded items and conformance with project plans and specifications. Deficiencies will be reported to the contractor for correction prior to the placement of concrete. If deficiencies are uncorrected, we will notify you and other designated persons of non-compliant conditions. Concrete Observation and Testing We will provide special inspectors certified by the ICC to observe concrete placements and sampling and field testing. Our field services include testing for slump, temperature (concrete and Task Order 10507.e Meridian WWTP Safety Improvement Project Special Inspections Page 2 of 6 STRATA ambient), air content, verification of mix design as indicated on the batch tickets, and fabricating of compressive strength test specimens, as required by the project specifications. Concrete sampling and field-testing is conducted by personnel certified by the American Concrete Institute (ACI) as a Concrete Testing Technician — Field Grade I. Please refer to the Estimate for anticipated amount of concrete test cylinders on this project. We have estimated these quantities based on IBC specifications, which require 1 composite sample and 1 set of compressive strength specimens for every 150 cubic yards (c.y.) placed. We will test 1 concrete compressive strength specimen at 7 days, 3 at 28 days, and a specimen will be held in reserve pending successful results of the 28 -day tests. STRATA will notify those person(s) identified by you via telephone should the test results of the 7 -day test achieve less than 70 percent of the specified compressive strength (f c), or if the 28 -day test results do not achieve f c. Hold specimens will be discarded if the 28 -day tests achieve f c, unless we receive specific instructions otherwise. Results of concrete compressive strength testing will be distributed via email to the distribution list established during the pre -construction meeting, unless directed otherwise. We based this Proposal on the assumption that compliance testing of other materials (i.e. aggregates, cement, etc.) will not be required. Therefore, we have not included these activities in our anticipated scope of services. If it becomes necessary, we can provide these services for an additional fee. Structural Steel Field Welding Observation We will provide special inspectors certified by either the American Welding Society (AWS-CWI) or ICC to observe welding operations during structural steel erection and connection at the project location. We accomplish this on a periodic basis as dictated by the approved drawings and applicable codes. Our services include the observation of welder certifications, material identification, joint fit -up, pre/post-weld heat treatment, and general compliance of structural steel construction to the project requirements and applicable codes. In -shop Structural Steel Fabrication Inspection STRATA anticipates the need for in -shop structural steel fabrication inspection based on our review of the project specifications. We will provide a special inspector certified by either the American Welding Society (AWS-CWI) or ICC to observe the in -shop fabrication process. This will be done on a periodic/continuous basis as dictated by the approved drawings and applicable codes. Our services include the observation of welder certifications, material identification, joint fit - up, pre/post-weld heat treatment, and general compliance of structural steel construction to the project requirements and applicable codes. STRATA can also provide non-destructive examination (NDE) ( i.e. PT, MT, UT) of welded connections if required, however we do not anticipate the need for NDE of welded connections for the in -shop fabrication portion of this project and have not included this service in our estimated budget. Post -Installed Anchors (Epoxy/Expansion Anchors) We will provide an ICC -certified special inspector to observe the installation of reinforcing steel/anchor bolts using a chemical epoxy, as specified. Our field services will include verification of the size, depth, spacing, and cleanliness of holes to receive the reinforcing steel/anchor. We will also verify adherence with the manufacturer's specification for the chemical epoxy anchor. If required, we can perform pull/load/torque-testing services of reinforcing steel bars/anchor bolts installed using the above referenced applications as part of our scope of services. Task Order 10507.e Meridian WWTP Safety Improvement Project Special Inspections Page 3 of 6 STRATA Laboratory Testing We anticipate that compliance testing of soil (moisture -density relationship curve, gradation) and concrete (compressive strength), will be required during construction. Technical Management & Reports Our Project Manager reviews the daily field reports generated by our field staff during construction. Items found in non-compliance with the project requirements are brought to the immediate attention of the construction superintendent. If the situation is not corrected, we will subsequently notify the designated design professional and those designated by the client. As re -inspection items can have a serious impact on our budget, our Project Manager will review the daily field reports to monitor items requiring re -inspection and the hours involved in these re -inspections. Upon project completion, we will provide a final summary report to document the materials testing and special inspection services conducted. TIME OF COMPLETION and COMPENSATION SCHEDULE Strata will provide our services on a time -and -expense basis in accordance with the rates established in the attached Estimate. These rates will remain in effect through the duration of the project. Overtime for our field staff will incur a 50% per hour increase to our standard rate for time in excess of 8 hours during one day (Monday through Friday), non-standard construction hours (before 8:00 AM or after 5:00 PM), and all hours on weekends and holidays. This Proposal excludes the cost of any equipment (i.e. scaffold, scissor lifts, telescopic booms, ladders, trench shoring) to gain access to any area requiring inspection or sampling of materials. In order to better service your project, Strata requests that you please schedule at least 24 hours in advance of the needed service. Calls for services with less than 8 hours notification may incur a Short Notice Fee equivalent to an overtime hourly rate. When Strata receives a short notice call, Strata will apply the overtime rate to all hours the short notice covers. Calls for service that are cancelled within 2 hours of the scheduled arrival time may incur a charge equivalent to 2 hours at the regular hourly rate. Our actual fees charged will be directly affected by the contractor's schedule, performance, work force, jurisdictional requirements and events which occur during the duration of the project which cannot always be predicted in advance. The Not -To -Exceed amount to complete all services listed above for this Task Order No. 10507.e is four thousand seven hundred twenty two dollars ($4,722.00). No compensation will be paid over the Not -to -Exceed amount without prior written approval by the City in the form of a Change Order. Travel and meals are excluded from this Task Order unless explicitly listed in the Scope of Services AND Payment Schedule.
