1. ALL CONSTRUCTION WORK SHALL BE DONE IN ACCORDANCE WITH THE CURRENT VERSION OF THE IDAHO STANDARDS FOR
PUBLIC WORKS CONSTRUCTION (ISPWC), THE CITY OF MERIDIAN SUPPLEMENTAL SPECIFICATIONS TO THE ISPWC, THE
REQUIREMENTS OF THE ADA COUNTY HIGHWAY DISTRICT (ACHD) AND/OR THE REQUIREMENTS OF THE IDAHO
TRANSPORTATION DEPARTMENT (IID). THE MORE STRINGENT OF ANY OF THESE STANDARDS SHALL BE 114E CONTROLLING
STANDARDS OR SPECIFICATIONS.
2. ALL THE CONTRACTOR SHALL HAVE PLANS STAMPED "APPROVED FOR CONSTRUCTION' BY CITY OF MERIDIAN PUBLIC WORKS
DEPARTMENT AND ACHD ON SITE AT ALL TIMES.
3, CONTRACTORS SHALL NOTIFY THE APPROPRIATE AGENCY WHEN MATERIALS ARE ON SITE OR INSPECTION OF THE WORK IS
REQUIRED. NO WORK MAY BEGIN ON ANY PROJECT WITHOUT TWENTY FOUR (24) HOUR PRIOR NOTICE.
4. ALL MATERIAL FURNISHED ON, OR FOR THE PROJECT MUST MEET THE MINIMUM REQUIREMENTS OF THE APPROVING
AGENCIES. AT THE REQUEST OF THE APPROVING AGENCY OR THE DESIGN ENGINEER, CONTRACTORS SHALL FURNISH PROOF
THAT ALL MATERIALS INSTALLED ON THIS PROJECT MEET THE SPECIFICATION REQUIREMENTS SET FORTH IN GENERAL
CONSTRUCTION NOTE NO. 1.
5. WORK SUBJECT TO APPROVAL BY ANY GOVERNMENTAL AGENCY MUST BE APPROVED PRIOR TO (A) BACKFILLING TRENCHES
FOR PIPE; (B) PLACING OF AGGREGATE BASE; (C) PLACING OF CONCRETE; (D) PLACING OF ASPHALT PAVING.
6. INSPECTION, APPROVAL AND FINAL ACCEPTANCE OF ALL WATER AND SEWER CONSTRUCTION SHALL BE BY THE PUBLIC
WORKS DEPARTMENT, AND THEIR DECISION SHALL BE FINAL. SUCH INSPECTIONS SHALL NOT RELIEVE THE CONTRACTOR FROM
THE RESPONSIBILITY OF PERFORMING THE WORK IN AN ACCEPTABLE MANNER IN ACCORDANCE NTH THE DEQ/QLPE
APPROVED CONSTRUCTION PLANS.
7. ANY DEVIATON FROM THE APPROVED PLANS AND SPECIFICATIONS MUST HAVE THE APPLICABLE AGENCY APPROVAL IN
WRITING PRIOR TO CONSTRUCTION.
B. PRIOR TO BEGINNING INSTALLATION OF STREET LIGHT, THE ELECTRICAL CONTRACTOR SHALL OBTAIN AN ELECTRICAL PERMIT
FROM THE BUILDING DMSION OF THE MERIDIAN PUBLIC WORKS DEPARTMENT. ANY DEVIATION IN STREET LIGHT LOCATIONS
FROM THE APPROVED PLANS MUST BE APPROVED IN WRITING FROM THE CITY ENGINEER.
9. TAKE ALL LOT DIMENSIONS, EASEMENTS AND CERTAIN OFF-SITE EASEMENTS FROM THE PLANS PROVIDED.
10. THE CONTRACTOR SHALL MAINTAIN ALL EXISTING DRAINAGE FACIUTIES WITHIN THE CONSTRUCTION AREA UNTIL THE
DRAINAGE IMPROVEMENTS ARE IN PLACE AND FUNCTIONING.
11. ALL CONTRACTORS WORKING WITHIN THE PROJECT BOUNDARIES ARE RESPONSIBLE FOR COMPLIANCE WITH ALL APPLICABLE
SAFETY LAWS OF ANY JURISDICTIONAL BODY. THE CONTRACTOR SHALL BE RESPONSIBLE FOR ALL BARRICADES, SAFETY
DEVICES AND CONTROL OF TRAFFIC WITHIN AND AROUND THE CONSTRUCTION AREA.
12. THE LOCATIONS OF EXISTING UNDERGROUND UTIUTIES ARE SHOWN IN AN APPROXIMATE WAY ONLY. THE CONTRACTOR
SHALL DETERMINE THE EXACT LOCATION OF ALL EXISTING UTILITIES BEFORE COMMENCING WORK. THE CONTRACTOR ASSUMES
ALL RESPONSIBILITY FOR ANY AND ALL DAMAGES CAUSED BY HIS FAILURE TO EXACTLY LOCATE AND PRESERVE ANY AND
ALL UNDERGROUND UTILITIES.
13. ALL CONTRACTORS WORKING WITHIN THE PUBLIC ROAD RIGHT-OF-WAY ARE REQUIRED TO SECURE A RIGHT-OF-WAY
CONSTRUCTION PERMIT FROM ACHD AT LEAST TWENTY-FOUR (24) HOURS PRIOR TO ANY CONSTRUCTION.
14. THE DESIGN ENGINEER SHALL SUBMIT RECORD AND ELECTRONIC DRAWINGS TO THE PUBLIC WORKS DEPARTMENT AS
PRESCRIBED IN THE MSSD SECTION 1.2.J.3 BEFORE FINAL APPROVAL IS GIVEN TO THE PROJECT.
15. ONLY PLAN SETS STAMPED "APPROVED FOR CONSTRUCTION" AND SIGNED BY THE CITY ENGINEER OR HIS AUTHORIZED
REPRESENTATIVE SHALL BE USED BY THE PROJECT CONTRACTOR(S). USE OF ANY PLANS ON THE JOB WITHOUT THE
"APPROVED FOR CONSTRUCTION" STAMP SHALL BE GROUNDS FOR THE ISSUANCE OF A STOP WORK ORDER.
16. LOCATE SUBSURFACE STORMWATER DISPOSAL FACILITIES (INCLUDING INFILTRATION BEDS AND DRYWELLS) AT LEAST 25 FEET
FROM WATER MAINS. THIS REQUIREMENT DOES NOT APPLY TO CATCH BASINS OR SAND AND GREASE VAULTS.
17. ANY COMMON LOT CONTAINING PUBLIC WATER OR SEWER MAINS SHALL BE FENCED AT THE LOT LINES.
18. QUANTITY AND PLACEMENT OF STREETLIGHTS ARE CONTINGENT UPON THE ELECTRICAL DESIGN BY IDAHO POWER COMPANY.
THE ELECTRICAL CONTRACTOR WILL BE PROVIDED A STREET LIGHT MAP BY THE MERIDIAN PUBLIC WORKS AND BUILDING
DEPARTMENT, WITH THE ISSUANCE OF AN ELECTRICAL PERMIT.
19. THE CONTRACTOR SHALL KEEP ON SITE AT ALL TIMES A COPY OF THE APPROVED CONSTRUCTION PLAN ON WHICH IS
RECORDED THE ACTUAL LOCATIONS OF THE CONSTRUCTED PIPE LINE AND ANY OTHER UTILITIES ENCOUNTERED. THE
CONTRACTOR SHALL PROVIDE THESE LOCATIONS TO THE DESIGN ENGINEER FOR USE IN THE PRODUCTION OF RECORD
DRAWINGS PER SECTION 1.2.J.3 PRIOR TO FINAL APPROVAL OF THE PIPELINE INSTALLATION.
20. ALL COSTS OF RETESTING FOR PREVIOUSLY FAILED TESTS SHALL BE BACK CHARGED TO THE CONTRACTOR BY THE OWNER.
21. ALL COSTS TO THE CONTRACTOR INCURRED IN CORRECTING DEFICIENT WORK SHALL BE TO THE CONTRACTORS ACCOUNT.
FAILURE TO CORRECT SUCH WORK WILL BE CAUSE FOR A STOP WORK ORDER AND POSSIBLE TERMINATION.
22. THE CONTRACTOR(S) SHALL REMOVE ALL OBSTRUCTIONS, BOTH ABOVE AND BELOW GROUND, AS REQUIRED FOR THE
CONSTRUCTION OF THE PROPOSED IMPROVEMENTS. THIS SHALL INCLUDE CLEARING AND GRUBBING WHICH CONSISTS OF
CLEARING THE GROUND SURFACE OF ALL TREES, STUMPS, BRUSH, UNDERGROWTH, HEDGES, HEAVY GROWTH OF GRASS OR
WEEDS, FENCES, STRUCTURES, DEBRIS, RUBBISH, AND SUCH MATERIAL WHICH, IN THE OPINION OF THE ENGINEER, IS
UNSUITABLE FOR THE FOUNDATION OF PAVEMENTS. ALL MATERIAL NOT SUITABLE FOR FUTURE USE ON SITE SHALL BE
LEGALLY DISPOSED OF OFF SITE.
23. BACKFILL OF LOTS AND/OR RELOCATED LATERALS SHALL REQUIRE INDEPENDENT SOILS TESTING LABORATORY. TO VERIFY
BOTTOM OF EXCAVATION PRIOR TO THE PLACEMENT OF STRUCTURAL BACKFILL TO ENSURE THAT UNSUITABLE SOILS HAVE
BEEN REMOVED. COMPACTION TESTING SHALL BE DONE AT EACH ONE FOOT OF VERTICAL LIFT AND EVERY 500 FT.
HORIZONTALLY. TESTING LOCATIONS TO BE ALTERNATED TO ENSURE UNIFORM COMPACTION ON EACH DEVELOPED LOT.
ENGINEERED BACKFILL TO BE COMPACTED TO A MINIMUM OF 90% OF MODIFIED PROCTOR PER ASTM D-1557 OR 95% OF
STANDARD PROCTOR PER ASTM D-698 TEST RESULTS TO BE SUBMITTED TO THE CITY OF MERIDIAN BUILDING DEPT. PRIOR
TO THE ISSUANCE OF ANY BUILDING PERMITS.
24. EACH CONTRACTOR SHALL BE RESPONSIBLE FOR ACQUIRING ANY NECESSARY NPDES PERMITS, FILING ANY N01'S, AND
PREPARING A STORMWATER POLLUTION PREVENTION PLAN (SWPPP)IN ACCORDANCE WITH THE ENVIRONMENTAL PROTECTION
AGENCY. CONTACT THE EPA AT 208-334-9489 FOR THE REQUIRED INFORMATION, SAID PERMIT SHALL BE PRESENTED TO
THE CITY ENGINEER AT LEAST 48 HOURS PRIOR TO THE BEGINNING OF CONSTRUCTION, AND A COPY MAINTAINED ONSITE.
25. MINIMUM SEPARATION FROM THE BOTTOM OF THE DRAINAGE FACILITY TO SEASONAL HIGH GROUND -WATER SHALL BE A
MINIMUM OF 3'. CONTACT DESIGN ENGINEER FOR A REVISED DRAINAGE FACILITY DESIGN IF BOTTOM OF FACILITY IS WITHIN 3'
OF SEASONAL HIGH GROUNDWATER ELEVATION. RE -DESIGNED FACILITY SHALL BE APPROVED BY PUBLIC WORKS
DEPARTMENT.
26. DRAINAGE FACILITIES, UTILITY POLES, LINES, AND APPURTENANCES WHEN NECESSARY TO REMOVE OR DISTURB, SHALL BE
REPLACED OR RECONSTRUCTED TO EQUAL OR BETTER TO THEIR CONDITON PRIOR TO DISTURBANCE.

LIDS, AND CONCRETE COLLARS, AND THE CONTRACTOR SHALL NOTIFY THE CITY OF MERIDIAN 24 HOURS PRIOR TO

9. ALL COSTS OF RETESTING FOR PREVIOUSLY FAILED TESTS SHALL BE BACKCHARGED TO THE CONTRACTOR BY THE OWNER.

19. THE ENGINEER OF RECORD CERTIFIES THAT THE PLANS ARE PREPARED IN SUBSTANTIAL CONFORMANCE WITH THE ACHD
POLICY AND STANDARDS IN EFFECT AT THE TIME OF PREPARATION. THE ENGINEER ACKNOWLEDGES THAT ACHD ASSUMES
NO LIABILITY FOR ERRORS OR DEFICIENCIES IN THE DESIGN. ALL VARIANCES FROM ACHD POLICY SHALL BE APPROVED IN

WITH THE ADA STANDARDS. SLOPES INCLUDING CROSS -SLOPE WILL BE VERIFIED TO MEET THESE STANDARDS AND THE
DISTRICT WILL ADHERE TO A STRICT "NO TOLERANCES" POLICY.
21. ACTUAL FIELD CONDITIONS DURING TRENCHING MAY REQUIRE ADDITIONAL PAVEMENT REPAIR BEYOND THE LIMITS SHOWN
ON THE PLANS. THE FOLLOWING CONDITIONS ARE LISTED IN SECTION 6000 OF THE ACHD POLICY MANUAL.
21.1. ALL ASPHALT MATCH LINES FOR PAVEMENT REPAIR SHALL BE PARALLEL TO THE CENTERLINE OF
THE STREET AND INCLUDE ANY AREA DAMAGED BY EQUIPMENT DURING TRENCHING OPERATIONS.
21.2. If THE CUMULATIVE DAMAGED PAVEMENT AREA EXCEEDS 50% OF THE TOTAL ROAD SURFACE, THE
CONTRACTOR SHALL REPLACE THE ENTIRE ROADWAY.
21.3. CONTRACTOR SHALL REPLACE THE PAVEMENT SURFACE TO ENSURE MATCH LINE DOES NOT FALL
WITHIN THE WHEEL PATH OF A LANE. MATCH LINE SHALL ONLY FALL IN THE CENTER OR EDGE OF
A TRAVEL LANE.
21.4. FLOWABLE FILL OR IMPORTED MATERIAL MAY BE REQUIRED IF THE NATIVE TRENCH MATERIAL IS
DEEMED UNSUITABLE BY ACHD INSPECTOR, DOES NOT MEET THE COMPACTION STANDARDS OR
TIME IS A CRITICAL FACTOR.
21.5, ANY EXCEPTIONS TO THESE RULES SHALL BE PRE -APPROVED IN WRITING BY DISTRICT STAFF
BEFORE CONSTRUCTION BEGINS.
22. THE CONTRACTOR CHOSEN TO INSTALL THE PAVER SYSTEM SHALL BE EXPERIENCED WITH PAVER INSTALLATION, AND
SHALL BE APPROVED BY THE PAVER SUPPLIER. THEY SHALL FOLLOW THE ICPI AND MANUFACTURERS TECHNICAL
SPECIFICATIONS AND RECOMMENDATIONS FOR THE INSTALLATION OF THE PAVER SYSTEM. SEE SHEET X.X FOR ADDITIONAL

AS THE STANDARDS AND SPECIFICATIONS REFERRED TO IN GENERAL CONSTRUCTION NOTE NO. 1.
2. THE HORIZONTAL SEPARATION OF NON -POTABLE SERVICES AND POTABLE SERVICES OR POTABLE WATER MAINS SHALL BE A
MINIMUM OF SIX (6) FEET. WHERE IT IS NECESSARY FOR A POTABLE WATER MAIN AND NON -POTABLE WATER MAIN TO CROSS WITH
LESS THAN EIGHTEEN (18) INCHES OF VERTICAL SEPARATION, THE CROSSING SHALL BE CONSTRUCTED IN ACCORDANCE WITH
SECTION 542.07 OF THE IDAHO RULES FOR PUBLIC DRINKING WATER SYSTEMS (IDAPA 58.01.08) AND SECTION 430.02 OF THE
WASTEWATER RULES (IDAPA 58.01.16).
3. PLACE SEWER SERVICE LINES IN A SIX (6) INCH DIAMETER WATER CLASS PIPE WHEREVER THE SERVICE LINE CROSSES A
STORMWATER TREATMENT FACILITY (I.E., SEEPAGE BEDS, DRAINAGE SWALES).
4. WHEN COVER OVER A SEWER PIPE IS LESS THAN THREE (3) FEET FROM TOP OF PIPE TO SUBGRADE OR TOP OF PIPE TO NATURAL
GROUND, USE "CLASS 200 WATER PRESSURE PIPE", ASTM D 2241, SDR 21, INCLUDING SERVICE LINES AND FITTINGS.
5. THE CONTRACTOR SHALL CONDUCT AN AIR PRESSURE TEST AND TELEVISION INSPECTON AEIM ALL UNDERGROUND UTILITIES HAVE
BEEN INSTALLED. THE CONTRACTOR SHALL PROVIDE A VIDEOTAPE OF THE INSPECTION PRIOR TO FINAL ACCEPTANCE OF THE SEWER.
6. APPROVAL AND ACCEPTANCE OF ALL SEWER CONSTRUCTION WILL BE BY THE PUBLIC WORKS DEPARTMENT AND THEIR DECISION
SHALL BE FINAL SUCH INSPECTIONS SHALL NOT RELIEVE THE CONTRACTOR FROM THE RESPONSIBILITY OF PERFORMING THE WORK

7. ALL SEWER PIPE SHALL BE BELL AND SPIGOT, POLYVINYL CHLORIDE (PVC), SDR 35, ASTM D-3034, UNLESS OTHER'MSE SPECIFIED.
ALL SEWER PIPE SHALL COMPLY WITH APPLICABLE PORTIONS OF SECTION 4.1 OF THE STANDARD SPECIFICATIONS AND DRAWINGS.
8. LOCATE SERVICE LINES TO THE POINTS SHOWN ON THE DRAWINGS OR AS MARKED BY THE ENGINEER IN THE FIELD. MARK AND
CONSTRUCT SERVICE LINES IN ACCORDANCE WITH THE STANDARD DRAWINGS 7.16(5) AND 7.17(S). THE SERVICE MARKER SHALL BE
IN PLACE FOR THE FINAL INSPECTION. SERVICE LINES SHALL EXTEND TEN (10) FEET BEYOND THE RIGHT -Of -WAY. SEWER SERVICE
LINES MAY BE A MAXIMUM FIVE (5) FEET DEEP AT THE PROPERTY LINE UNLESS OTHERWISE APPROVED BY CITY ENGINEER.

1. ALL DRAINAGE FACILITIES MUST BE INSPECTED BY CITY OF MERIDIAN PUBLIC
WORKS AND OWNERS ENGINEER. 24 HOUR NOTICE IS REQUIRED.
2. SEEPAGE BED MUST BE 5 FEET BACK FROM PROPERTY LINE AND NO GRADING
IS PERMITTED WITHIN 2 FEET OF PROPERTY UNE.
3. PER THE SOILS GEOTECHNICAL REPORT, GROUNDWATER NOT TO 15' BELOW THE
SURFACE.
4. SEPARATION FROM THE BOTTOM OF THE DRAINAGE FACILITY TO SEASONAL HIGH
GROUNDWATER SHALL BE A MINIMUM OF 3'. CONTACT ENGINEER FOR A REVISED
DRAINAGE FACILITY DESIGN IF BOTTOM OF FACILITY IS WITHIN 3' OF SEASONAL
HIGH GROUNDWATER ELEVATION. RE -DESIGNED FACILITY SHALL BE APPROVED BY
THE PUBLIC WORKS DEPARTMENT.
5. ANY CHANGE FROM THE PLANS SHALL BE APPROVED BY THE DESIGNER AND
THE CITY OF MERIDIAN PUBLIC WORKS DEPARTMENT.
6. THE CONTRACTOR SHALL CONTACT THE CITY OF MERIDIAN PUBLIC WORKS
DEPARTMENT FOR INSPECTION OF ALL STORM WATER FACILITIES PRIOR TO THE
PLACEMENT OF FILTER FABRIC. MINIMUM OF 24-HOUR NOTICE IS REQUIRED.
