Idaho Ave.TASK ORDER NO. 10275a Under the MASTER AGREEMENT FOR PROFESSIONAL SERVICES BETWEEN CITY OF MERIDIAN (OWNER) AND CIVIL SURVEY CONSULTANTS, INC This Task Order is made this (sT day of _K%—X4 t, 2011 and entered into by and between the City of Meridian, a municipal corporation organized under the laws of the State of Idaho, hereinafter referred to as "City", and accepted by CIVIL SURVEY CONSULTANTS, INC, hereinafter referred to as "Engineer" pursuant to the mutual promises, covenant and conditions contained in the Master Agreement (Category 1 B) between the above mentioned parties dated April 13, 2010. The Project Name for this Task Order 10275a is as follows: WATER SERVICE REPLACEMENT (DESIGN) — W. IDAHO AVENUE SCOPE OF SERVICES Per the City's request, the Engineer will provide professional engineering services related to the design of replacing the existing water service at 403, 417, 420, 427, 428, 437, 438, and 500 W. Idaho Avenue. The design will include abandoning the existing 8 services and replacing them with a new 1" meter and water service line from the main to the meter. The existing service line on the property side of the meter will be reconnected to the new meter. Task 1 – Administration 1.1 This task includes duties associated with setting up the contract and submitting monthly invoices. Task 2 - Design Surveying 2.1 This task includes field work associated with performing topographic survey required to complete the design. Engineer will request utility maps from the utility companies. 2.2 This task includes office work associated with drafting the topographic survey and establishing the public right-of-way and property lines. We anticipate that for the purposes of this project we will be able to define the existing public right-of–way and property lines from previous projects done in the area for the City and from record of surveys and subdivision plats recorded with Ada County. Task 3 - Design Water Service 3.1 This task includes work associated designing the new water services. 3.2 Send a letter to property owners notifying them of the City's intention to replace their Page 1 of 3 water service and the projects anticipated construction schedule. The letter will be submitted to City staff for approval prior to the mailing of the letter. City will provide property owner mailing addresses as needed. This task also includes time responding to telephone calls from property owners. Task 4 — Construction Document Preparation and Review 4.1 Develop plans and specifications and bid schedule that meet current design standards and standard specifications of the City and ISPWC. 4.2 Submit 2 full size physical copies of plans and special provisions to City Engineering for Development Services review and approval, Engineer to revise plans as necessary and resubmit. 4.3 Submit 4 full size physical copies of plans and special provisions to City Engineering for QLPE review and approval, Engineer to revise plans as necessary and resubmit. 4.4 Submit two sets of plans to ACHD for review and approval prior to construction. Includes one set of revisions per ACHD comments and then resubmitting to ACHD for approval. 4.5 Prepare a Traffic Control Plan and submit the plan to ACHD construction services for review and approval. Engineer to revise plans per ACHD comments and resubmit. 4.6 Prepare an Erosion and Sediment Control Plan (ESCP) and submit the plan to the City and ACRD for review and approval. The ESCP will be prepared in accordance with the City of Meridian Construction Stormwater Management Program. All other stormwater management program requirements shall be completed by the City or the Contractor. 4.7 Prepare final construction documents with inclusion of any addenda from the bidding phase and provide one physical copy for City's use and 10 physical copies for contractor's use. Task 5 — Construction Services 5.1 Provide one set of construction stakes for the new water services. It is anticipated that one point will be staked for each service. Staking will be completed as requested by the City and Contractor. 5.2 Attend pre -construction meeting. 5.3 Answer RFI's from contractor during bidding and construction about the design. 5.4 Review material specifications submitted by contractor. 5.5 Prepare record drawings meeting the City of Meridian Acceptance of Record/Electronic Drawings, dated August 26, 2009. Page 2 of 3 TIME OF COMPLETION Engineer will work with project manager to complete design in a timely manner. COMPENSATION The Not -To -Exceed amount for this Task Order No. 10275a is Six Thousand Five Hundred Sixty - Eight Dollars ($6,568.00). The hourly rates for services and direct expenses is per the Master Agreement and by this reference made a part hereof. Costs will be billed for actual time services are rendered up to the Not -to -Exceed amount.
