HomeMy WebLinkAboutTask Order 10160b with Strata, Inc. for WWTP Tertiary Filters BuildingTASK ORDER NO. 101608 Under the MASTER AGREEMENT FOR PROFESSIONAL SERVICES BETWEEN CITY OF MERIDIAN (OWNER) AND STRATA, INC (CONSULTANT) This Task Order is issued by City and accepted by the Consultant pursuant to the mutual promises, covenant and conditions contained in the Master Agreement (Category 4A) between the above mentioned parties dated June 27, 2011. CITY OF MERIDIAN WWTP TERTIARY FILTERS BUILDING (MATERIALS TESTING AND SPECIAL INSPECTION SERVICES) PURPOSE To provide testing and inspection services for the WWTP Tertiary Filters Building Project as follows: 1) Geotechnical observation and density testing during site grading and utility trench backfill; 2) Observation and compaction testing during the placement of hot -mixed asphalt (HMA) pavement; 3) Special inspection of reinforcement steel and embedded items prior to concrete placement; 4) Sampling and field testing of concrete; 5) Special inspection during the placement and consolidation of structural concrete; 6) Special inspection of structural steel construction (high-strength bolting); 7) Special inspection and testing of non -shrink grout; 8) Special inspection during the installation of drilled -in -anchors (chemical epoxy or expansion type anchors), and; 9) Sampling and laboratory testing of various construction materials. SCOPE OF WORK Consultant will provide a Project Manager to supervise Consultant's technical staff. Project Manager will coordinate and schedule all special inspections, field observations and laboratory services. Technical staff shall be certified testing and special inspection professionals. The Project Manager will also serve as the liaison between the City and General Contractor. Consultant will assign a special inspector, with multiple certifications and experience, who will provide the majority of the special inspections and testing on the project. Lab testing will occur at Consultant's accredited lab and Consultant shall supply verification of accreditation to City. Consultant shall provide transportation of all samples to and from the lab for testing. Deliverables Reports of laboratory testing will be distributed as established by the City in the respective pre-construction/pre-installation meetings WWTP Filters Building Testing & special Inspection Project No. 101608 Page 1 of 5 Pre -Construction Meetings Consultants project manager shall attend a pre -construction meeting with the General Contractor and the City Project Manager to review the special inspection and materials testing requirements for the project. Consultant shall also attend subsequent pre -installation meetings with general contractor and sub- contractors. Deliverables: Following the pre -construction meeting Consultant shall produce an MOU of Consultant's roles and responsibilities, and inspection and testing personnel qualifications. Geotechnical Evaluation, Observation, and Testing Services Consultant shall provide field personnel to perform observation and compaction testing services during site preparation/grading and backfilling of utility trenches as established in the project geotechnical evaluation documents (plans, specifications, and consultanting evaluation reports). Compaction testing will be performed as required by the project specifications and the geotechnical report. Consultant will sample any on-site and imported materials used for backfill. Deliverables: Reports of laboratory testing will be distributed as established by the City in the respective pre-construction/pre-installation meetings. Hot -Mixed Asphalt (HMA) Paving Observation and Testing Consultant shall observe the placement and preparation of base material prior to HMA placement. Consultant shall monitor the placement and compaction efforts as required by the project documents. Consultant shall sample and test HMA. Consultant shall extract fore specimens of the HMA to verify in-place thickness and density. Deliverables: Reports of laboratory testing will be distributed as established by the City in the respective pre-construction/pre-installation meetings. Special Inspection of the Placement of Reinforcing Steel and Embedded Items Consultant shall provide an International Code Council (ICC) certified special inspector to verify the placement of reinforcing steel and embedded items prior to the placement of concrete. Inspection of reinforcing steel and embedded items will include verification of type, grade, size, cleanliness, lap splice, clearance, coverage, general placement of reinforcing steel/embedded items and conformance with project plans and specifications. Any discrepancies will be reported to the contractor for correction prior to the placement of concrete. Deliverables: If discrepancies are uncorrected, Consultant will notify City and other designated persons of non-compliant conditions in writing within 24 Hours. Special Inspection and Testing of Concrete Consultant shall provide an ICC -certified special inspector to provide observation during concrete placement and consolidation operations. Consultant will perform sampling and field testing of concrete WWTP Filters Building Testing & special Inspection Project No. 10160B Page 2 of 5 including casting of compression test cylinders for strength verification purposes. Field services will include testing for slump, temperature (concrete and ambient), air content, verification of mix design, and casting of compression test specimens. All sampling and field-testing of concrete will be conducted by personnel certified by the American Concrete Institute (ACI) as a Concrete Field Testing Technician - Grade I Consultant shall conduct one set of five compression test cylinders for each day's pour of 150 cubic yards or once for each day's placement. One 4 -inch by 8 -inch laboratory cured compression test cylinder will be tested at 7 days age, 3 will be tested at 28 days age, and the remaining cylinder will be held for future testing if necessary. Hold specimens will be discarded if the 28 -day tests achieve design strength (f'c) unless we receive specific instructions otherwise. Additional compression test cylinders may be required depending on the contractors scheduling of concrete placement. Deliverables: Consultant shall notify the City and the general contractor via telephone should the test results of the 7 -day test achieve less than 70% of the design strength (f'c). Results of concrete compressive strength testing will be distributed via email to the distribution list established during the preconstruction meeting, unless directed otherwise. Structural Steel High -Strength Bolting Observation and Testing Consultant shall provide an ]CC -certified special inspector to observe the installation and tensioning of high strength bolts as dictated on the approved project plans. In addition to a visual inspection to verify the proper tensioning, Consultant will perform tension verification testing on 100% of the high strength main structural bolted connections. Consultant shall also provide continuous observation any bolts tightened using the turn -of -nut method. Deliverables: Reports of laboratory testing will be distributed as established by the City in the respective pre-construction/pre-installation meetings. Observation and Testing of Non -Shrink Grout Consultant shall provide a special inspector certified by the ICC to observe the placement of non -shrink grout as indicated on the project plans. Consultant's field services will include verification of the grout space, substrate condition, and the cleanliness of areas to receive non -shrink grout. Consultant will also verify adherence with the manufacturer's specification for the mixing, placement, and consolidation of non - shrink grout. Additionally, Consultant will sample the non -shrink grout and cast 1 set of 3 specimens (2"x2Ux2" cubes) for verification of compressive strength of non -shrink grout. The frequency of sampling will be performed as specified in the project documents. Deliverables: Reports of laboratory testing will be distributed as established by the City in the respective pre-construction/pre-installation meetings. Special Inspection of Drilled -In -Anchors (Epoxy/Expansion Anchors) Consultant shall provide an ICC -certified special inspector to observe the installation of reinforcing steel/anchor bolts using either a chemical epoxy or expansion type anchor, as specified. Consultant's field services will include verification of the size, depth, spacing, and cleanliness of holes to receive the reinforcing steel/anchor, Consultant will also verify adherence with the manufacturer's specification for the chemical epoxylexpansion anchor. Consultant will perform pull/load/torque-testing services of 10% of reinforcing steel bars/anchor bolts installed using the above reference applications. wWTP Filters Building Testing & Special Inspection Project No. 101600 Page 3 of 5 Deliverables Reports of laboratory testing will be distributed as established by the City in the respective pre-construction/pre-installation meetings. Technical Management & Reports Consultant's project manager will review the daily progress reports generated by our field staff during construction. Consultant's project manager will also review the daily progress reports to monitor items requiring re -inspection and the hours involved in these re -inspections Deliverables: Items found in non-compliance with the project requirements will be brought to the immediate attention of the contractor for correction; additionally we will notify the designated design professionals and your project representative. Re -inspection items will be documented and this information will be forwarded to City periodically for appropriate action, and tracking of potential change orders and/or back charges. Upon completion of the project, Consultant will provide a final summary report to document the construction materials testing and special inspection services conducted. Administrative time to mobilize and set-up your project to accommodate monthly itemized invoicing and reporting of our findings has been included in our Estimate. If additional supporting documentation or specialized invoicing is required, additional administrative time may be required to accommodate your request. TIME OF COMPLETION The time to complete this task order will follow the construction schedule as outlined by the contractor. COMPENSATION The Nat -To -Exceed amount to complete all tasks for this Task Order No. 10160B is sixteen thousand three hundred fifty-one dollars ($16,255.60) per the attached Fee Schedule. Administrative time to mobilize and set-up the project to accommodate monthly itemized invoicing and reporting of Consultant's findings is included in the Nat -To -Exceed amount. The hourly rates for additional services and any direct expense will be billed per the Master Agreement and by this reference made a part hereof.
