Under the
MASTER AGREEMENT FOR PROFESSIONAL SERVICES
BETWEEN
CITY OF MERIDIAN (OWNER) AND SPF WATER ENGINEERING, LLC
This Task Order is issued by City and accepted by the Engineer pursuant to the mutual promises,
covenant and conditions contained in the Master Agreement 6A between the above mentioned
parties dated April 13, 2010.

SCOPE OF WORK
TASK 10011i — ENGINEERING SERVICES FOR WELL 10B DESIGN
The City turned off Well 10 in August or September 2008 in response to uranium concentrations
exceeding the primary drinking water standard. Prior to being turned off, Well 10 provided
approximately 1,100 gpm to Zone 2 of the City's municipal water system. The City intended to
reconstruct Well 10 by pulling the screens from the existing well and constructing a new well inside
Well 10. However, the screen in Well 10 was not successfully removed, and the City will abandon
the well and construct a new well on the same lot. SPF's scope of services shall include the
following subtasks.
Subtask 1. Well 10 Abandonment Oversight. SPF will continue to support the City with
abandonment of Well 10. SPF will provide field oversight of the abandonment of Well 10. SPF will
assist the City to obtain a permit from IDWR to abandon Well 10 (once a Contractor is selected for
the abandonment work). SPF will document the abandonment in the form of a memorandum that
will be delivered to the City within one to two weeks following the completion of the abandonment
work.
Subtask 2. Well 108 Design and Permitting. SPF will prepare a Well Preliminary Engineering
Report, Well Site Evaluation, and Well Plan and Specifications for submittal to IDEQ. The
Engineer will respond to IDEQ comments on the submittal.
Subtask 3. Well 10B Bidding, Construction Management, and Well Completion Report. SPF
will assist the City in obtaining and reviewing bids for drilling and construction of the production
well. SPF will provide technical specifications and a bid schedule, will evaluate bids, and will
provide a recommendation for awarding the work based on contractor cost and qualifications. SPF
will coordinate with the contractor during well construction and test pumping. SPF will provide
construction management services including examination of drill cuttings, drilling fluid monitoring,

final well design, inspection of the annular seal, supervision of well development and test pumping,
water quality sampling, and review of pay requests. SPF will prepare a Well Completion Report for
submittal to IDEA. The report will include an as -built well construction drawing, lithologic and
geophysical logs (obtained from the test well on site), water quality data, test pumping results,
analysis of well capacity, and recommendations for pump selection.
TIME OF COMPLETION
SPF will complete Subtask 2 (well design and permitting) within 30 days of the City's notice to
proceed. SPF will complete construction management services in conjunction with the City's
bidding and construction schedule, subject to contractor availability and IDEA review time. SPF
will complete the well completion report within 30 days of receiving water quality laboratory data.
COMPENSATION
The Not -to -Exceed amount for this Task Order No. 10011 is $24,200 (twenty-four thousand two
hundred dollars) based on the following budget for each Subtask.

The hourly rate for services and direct expenses is per the Prevailing Fee Schedule, Effective
January 1, 2011, and by this reference made a part hereof. Costs will be billed for actual time
services are rendered up to the Not -to -Exceed amount. No compensation will be paid over the
Not -to -Exceed amount without prior written approval by the City in the form of a Change Order.
Compensation includes laboratory fees for water quality analysis. Water quality samples collected
from the finished production well will be analyzed for all regulated primary and secondary water
quality parameters.
Compensation does not include contractor fees for drilling, construction, and test pumping. The
City will contract directly with the contractor(s) for their services. Compensation does not include
design, permitting, or construction management services associated with the pump and all other
appurtenances for the new well and well house.
