Under the
MASTER AGREEMENT FOR PROFESSIONAL SERVICES
BETWEEN
CITY OF MERIDIAN (OWNER) AND CIVIL SURVEY CONSULTANTS, INC (ENGINEER)
This Task Order is made this I JTT day of November, 2011 and entered into by and between the
City of Meridian, a municipal corporation organized under the laws of the State of Idaho,
hereinafter referred to as "City", and accepted by CIVIL SURVEY CONSULTANTS, INC,
hereinafter referred to as "Engineer" pursuant to the mutual promises, covenant and conditions
contained in the Master Agreement between the above mentioned parties dated April 13, 2010.
The Project Name for this Task Order 10049b is as follows:
WASTEWATER TREATMENT PLANT — MISCELLANEOUS ASPHALT
REPAIR TOPOGRAPHIC MAPPING SURVEYS
SCOPE OF SERVICES
Surveyor will provide professional surveying services related to the design of asphalt repair for four
(4) locations at the Wastewater Treatment Plant totaling approximately 32,000 square feet. The
services are at the direction of SPF Water Engineering.
Task 1 — Topographic Survey
1.1 This task includes providing field surveys to locate edge of pavement, nearby facility
features and spot elevations at an approximate 25 foot grid for use by SPF Water
Engineering to generate a surface and contours.
DELIVERABLES
This task includes drafting of topographic mapping of field surveys and preparing data for
submission to SPF Water Engineering.
TIME OF COMPLETION
Surveying services will be completed by November 30, 2011.
COMPENSATION
The Not -To -Exceed amount for this Task Order No. 10049b is One thousand seven hundred
eighty-eight dollars ($1,788.00).
The hourly rates for services and direct expenses are per the Master Agreement and by this
reference made a part hereof. No compensation will be paid over the Not -to -Exceed amount
without prior written approval by the City in the form of a Change Order. Travel and meals are

excluded from this Task Order unless explicitly listed in the Scope of Services AND Payment
Schedule.
