The following resolutions to drilled pier locations were agreed upon by City, Engineer and
Contractor. Contractor to proceed with drilling piers as outlined below. (Pier location numbers per attached

Foundation Revisions per Design Clarification #4 issued on 814111 by CH2MHill and per attached pricing dated
8119111.
The Prime Contractor, The Ewing Company and the subcontractor, Robert P. Jones Company shall not be responsible for
any direct, indirect, incidental, or consequential damages occurring as a result of the excavation of the drilled pier sites
for the Meridian WWTP Filters Building Project to equipment, structures, or material located below the surface of the
ground which are not shown on the plans or marked in the field, and which are within the location of the sites for each of
the drilled piers. Disclaimer shall only be inclusive of work completed in accordance with industry -accepted standards of
caution and workmanship.
In the event there is such an occurrence, and the city requests the Robert P. Jones Company to standby, Robert P. Jones
shall be compensated for a standby time of a 4 man crew at $125.00/hour. If standby time lasts longer than 1
hour, subcontractor shall move to a different drilled pier site to resume operations while a repair or resolution is being
completed.

We re pleased to quote the following lump sum price for the drilled piers for the
Meridian waste Water Treatment Filter Buildings including Addendum #1, 02, #3. This
quote is per the plans and specifications of CH2M Hill dated April 11, 2011 plus changes
since August 11, 2011.
This price includes mobilization, demobilization, all excavation for drilled piers,
removal of soil from site, furnishing all casing, dewatering, extraction of casing, placing
rebar cages, placing of concrete, furnishing concrete, famish rebar cages and submittal of
all reports.
The prime contractor to locate center of drilled piers, coordinate location of all
utility lines, famish permits and all testing.
Our lump sum price for the drilled piers as listed in the bid documents is as
follows:
Filter Building (A) 13 - 36" dia. drilled piers........................$17,368.00
Filter Building (B) 7 — 36" dia. drilled piers ........................ • 8,517.00
Total Lump Sum Price ................................................ $25.885.00

Cap the scum line north of the drilled pier location and pour thrust block to safeguard the line.
Repair electrical lines that were damaged and relocate to the north near the asphalt cut to avoid
further damage during drilling of the pier.
