The Contractor is hereby directed to make the following changes from the Contract Documents and Plans.
Description:
Evaluate realignment options, additional survey staking and review of submittals relating to re-
alignment options.
Reason for Change Order:
The city looked at several options to re -align the sewer design and JUB assisted from a technical
point. The city also authorized more survey staking to facilitate sewer line installation and the
contractors ability to install the line closer to tolerance than normal due to depth and flat grades. JUB
will need to review several submittals regarding changes that were made to the original installation that
will ultimately save the city money on the project.

This Task Order is issued by City and accepted by the Engineer pursuant to the mutual promises, covenant and
conditions contained in the Master Agreement between the above mentioned parties dated April 13, 2010,
VICTORY ROAD CONNECTOR DESIGN SERVICES
SCOPE OF SERVICES
1) Assist the CITY in evaluating realignment options to avoid the power poles east of the Kennedy Lateral.
2) Set horizontal and vertical control at 100' intervals and recalculate and re -stake the site based on moving
Manhole #1.
3) Review the contractor's plan to remove the existing lift station and wet well and move manhole #4 North
approximately 15'. Review contractor submittals (2) for the substitution of inside drops at Manholes #2
and #4.
TIME OF PERFORMANCE
J -U -B will perform the above noted work at the CITY's request. Per the original staking agreement, the contractor
will provide 48 hours notice for staking requests. J -U -B will review and respond in writing to the above noted
contractor submittals within 24 hours.
COMPENSATION
Create a new task 3.5 Additional Services — with a total not to exceed budget of $3,710.00, broken down as
follows:
1) $750.00
2) $2400.00
3) $560.00
