CONTRACT CHANGE ORDER
CONTRACTOR: CIVIL SURVEY CONSULTANTS, INC.
PROJECT: WATERLINE REPLACEMENT DESIGN - E. WILLIAMS ST. - E. KING ST.
The Contractor is hereby directed to make the following changes from the Contract Documents and Plans.
Description: Include sewer and water improvements in Main St. between Franklin Rd. and Ada St. as part of the
construction plans. Modify plans, specifications, bid schedule and construction estimate to include improvements
on Main St. Provide plans, specifications, bid schedule and construction estimate to ACHD for use in the overlay
project. Record drawings shall be expanded to include sewer and water improvements in Main St. Additional
construction staking shall be completed for water and sewer improvements in Main St, ALL OTHER TERMS
AND CONDITIONS REMAIN THE SAME.
Reason for Change Order: ACHD has changed the scope of a roadway project which w II now directly impact
the City's planned projects for the south end of downtown. As a result, future improvements must be designed
this fiscal year to allow construct. on in FYI 2. See attached letter from Civil Survey for additional information.
