Description: Civil Survey Consultants (CSC) original contract did not include the following construction staking items that were required to construct the project as revised after original Task order was approved: 1. Horizontal and vertical control of the reuse line at 25 crossings with future storm drain and irrigation lines. The reuse line is being installed to avoid conflicts with future storm drain and irrigation lines. 2. Horizontal and vertical control of the new asphalt pathway that is replacing the concrete sidewalk that was removed to allow for the installation of the reuse line. Reason for Change Order: CSC original contract did not staking items (as noted in item description) that were required to construct the project as revised. Attachments: 1. Broadway Avenue, Ste. 200 Meridian, ID 83642 Re: Ten Mile Road Reuse Line - Additional Construction Services Task Order No. 0642C Dear John: Our original contract with the City of Meridian under Task Order No. 0642C involved providing professional services to install a reuse main in Ten Mile Road in conjunction with the Ada County Highway District's (ACHD) rebuild of Ten Mile Road. ACRD rescheduled the rebuild of Ten Mile Road for 2015 and the City determined that they had a desire to have this reuse line operational this summer and the reuse line is currently being installed_ Our original contract did not include the following construction staking items that were required to construct the project as designed: Horizontal and vertical control of the reuse line at 25 crossings with future storm drain and irrigation lines. Under the original contract the storm drain and irrigation crossings would have been staked and installed by ACHD as part of their rebuild project allowing the contractor to install the reuse line with the necessary vertical separation from the storm drain and irrigation crossings. As part of the current project, it is necessary for us to stake the location of the future storm drain and irrigation crossings to allow the contractor to install the reuse line with the necessary vertical separation from the future crossings. 2. Horizontal and vertical control of the new asphalt pathway that is replacing the concrete sidewalk that was removed to allow for the installation of the reuse line. The existing sidewalk is located within the limits of the future roadway and will be removed during ACHD's future rebuild of Ten Mile Road.
