RE: Task Order 061OC: Ten Mile Road (Franklin to Cherry Lane) Reuse and Sewer Main Design —
Construction Addendum
We have attached for your review a proposed authorization for additional services which adjusts the
contract amount on the above referenced project. Our justification to increase the budget over our
original estimate is outlined below:
1) Staking Services
• As you know, estimating the level of effort for construction support services is difficult,
primarily due to the inherit nature of construction, contractor's capabilities, and difficulties
encountered during the project. To date we have been called out to stake 20 times over the
course of the project, and there remains more staking to be completed on the Ten Mile
reuse line. This level of staking was not anticipated in our estimate.
• Some re -staking was required due to communication/staging issues between roadway and
pipeline contractors, and flooding issues on Ten Mile Creek.
• We were requested by the contractor to calculate proposed finished street grades. This
requires more effort than what we normally anticipate when completing staking project in
existing streets. Consequently, our field survey and technician time is over our original
budget estimate.
• Scope of the project increased some with the addition of stub -outs and services.
• Staking at 100' intervals has been requested by the contractor.
2) Construction Administration and Additional Design
• There were additional service connections and stub -outs design during construction that
required revisions to the construction plans. Specifically, plan sheets 103A, 104A, 107A, and
501A were revised. This design work was billed to construction services.

The above table summarizes the original budget and our current charges to date on the project.
Approximately, $1200 was moved over from the agency permitting contract amount to help add in
the extra design work, but we have incurred approximately $5,000 in costs over the original
construction services estimate. We are requesting consideration of increasing the budget an
additional $7,600 to finish completion of the record drawing and remaining staking on the reuse
pipeline. Please review this proposal and feel free to contact me if you have any questions or
concerns. If this is acceptable, we will prepare an addendum for signature. Thank you for your

ACHD ROAD PROJECT WITH JUB ENGINEERS FOR A NOT TO
EXCEED AMOUNT OF $7,600.00 IN ACCORDANCE WITH THE
MASTER AGREEMENT DATED JANUARY 1, 2006.
L RECOMMENDED ACTION
A. Approve the Task Order 0610C for Additional Services No. 2 with JUB
Engineers for the Ten Mile Road -- Franklin Rd to Cherry Lane Improvement
Project in Conjunction with ACHD Project Number 504003 for a not to exceed
amount of $7,600.00.
B. Authorize the Purchasing Department to issue a purchase order in the amount of
$7,600.00.

IIY. DESCRIPTION
A. Background
Construction staking has taken more time than was included in the original scope
of work due to the scattered and disconnected locations of our utilities within this
project. The nature of the work requiring those was unknown when the original
scope was developed.
The City also requested JUB to add several utility extensions to the project, which
required additional design and construction surveying. Extra work is also required
to increase the amount for record surveying and drafting required by these
additional features.
B. Proposed Project
Attached is an Authorization for Additional Services No. 2 Task Order with JUB
Engineers for the additional design of City requested utility additions,
construction staking, record surveying, record drawing production of those
additions and additional staking required due to the complexity of this project.
Please see attached documentation from JUB regarding these additional requested
costs. This addendum is for $7,600.00.
IV. IMPACT
A. Strategic Impact:
This project meets our mission requirements to indentify and prioritize work to
anticipate, plan and provide public services and facilities that support the needs of
our growing community, ensuring modern reliable facilities while maintaining
financial stewardship.
This project will complete approximately half of the pipeline required to serve the
Ten Mile Interchange area with Class "A" reclaimed water.
B. Service/Delivery Impact:
This Task Order will compensate JUB for the extra work required to complete
this project and allows for an increase in the amount for the record surveying and
drafting required by those additional features indicated in Item III above.
C. Fiscal Impact / Project Costs
The funding requested for this project is a not to exceed amount of $7,600.00 and
will bring the total project Design and Construction Assistance costs to
$61,045.00.
